What is lost between what a patient says and what a chart holds?
Every clinical conversation carries more than its transcript. The symptom that keeps returning. The worry that gets minimized. The detail that changes over time. The thing never said the same way twice. Most of it disappears before it reaches a decision.
Signal Literature Health is building on that problem: measuring what conversation carries, what practice loses, and what becomes possible when it is held.
The work joins two disciplines that rarely meet: the measurement of language as structure, and the clinical judgment of what that structure means for care.

Joe Trabocco · Founder & CEO, Signal Literature, Inc.
Originator of Linguistic Coherence Architecture and the Coherence Architect role on the human side of generative AI. His work investigates how structure in human language can be recognized, preserved, and carried across human-AI interaction, leading to the development of AXIS, a coherence-governance architecture for extended generative AI exchange.
Trabocco’s research draws on more than ten thousand interactions across frontier AI systems and a published body of more than 250 papers spanning linguistic coherence, interaction behavior, and human-AI systems. His formally named frameworks include In-Session Behavioral Impact (ISBI), the bounded study of how interaction itself alters model behavior within a single session, and Premature Containment, a sequencing failure in which systems narrow coherent input before demonstrating they have understood it. These form part of a broader body of more than a dozen named frameworks developed through his work, now independently surfacing in retrieval across Google and major AI systems. He brings three decades of entrepreneurial and business leadership. Trabocco is also the author of eight books of fiction, poetry, and philosophy.

Dr Shivam Natarajan · Co-Founder & Managing Director, Signal Literature, Inc.
Senior healthcare transformation leader, clinician, and digital health strategist with more than 25 years of international experience across the NHS, the Middle East, Europe, and the global health technology sector. A surgeon by training with an MBA, he currently serves as Global Head of Healthcare and Lifesciences at Experion Technologies, and has held senior roles including Chief Medical Information Officer at Dedalus Group, Chief of Healthcare Services at Ajlan Medical Sector in Saudi Arabia, Senior Advisor and Head of eHealth to the Ministry of Health of Saudi Arabia, and transformation leadership across major NHS organisations and Integrated Care Systems across England, including Barts Health and Royal Free London.
Dr Natarajan has led national-scale programmes in electronic patient records, AI governance, population health, and integrated care, with procurements exceeding hundreds of millions of pounds. He is the founder of NewMedSkills, an AI-enabled platform for upskilling healthcare professionals, and a recognised international voice on responsible AI in medicine.
Both Trabocco and Dr Natarajan have been recognized for pioneering work in AI, from opposite sides of the same problem: one in how language governs machine behavior, the other in how machines enter medicine.
